How to make a lead form and a QR code

Build a form that puts inquiries straight into your pipeline, print a QR code for a sign or flyer, see where your leads come from, and reward customers who send customers.

Updated 2026-10-18

Forms & QR is where your leads come from: web forms, QR codes on signs and flyers, and customers who send customers. It is under Growth → Forms & QR. (If it is not in the menu, switch it on under Settings → What you use.)

Across the top you see how many Forms you have, how many were Sent in 30 days, your QR codes and their Scans. The page has four tabs: Lead sources, Forms, QR codes and Referrals.

Only someone who manages the business can build forms, make codes or change the reward.

Make a form

  1. Open the Forms tab and press New form.
  2. Change the Title if you like. It starts as "Get a free estimate".
  3. Under Fields, the form starts with Your name, Email, Phone, What do you need? and a box to agree to hear about offers and news. Change any question's wording, tick Needed for the ones people must fill in, or press the bin to remove one.
  4. Add more with the buttons under the fields: Name, Email, Phone, Address, Choice list, Free text or Agree to hear from us. For a Choice list, type the choices separated by commas.
  5. Choose where New leads land in: a stage of your pipeline, or the first stage.
  6. Tick who to Email these people when someone sends it.
  7. Add a Thank-you message if you want one, for example "Thanks — we'll call you within a day."
  8. Press Save.

Press the form again to see its address. Press Link to copy it, or Embed code to copy the code that puts the form on your own website or anyone else's. Leads from the form get it as their source, shown under the address. Switch Form is live off to stop it taking inquiries for a while.

Make a QR code

  1. Open the QR codes tab and press New QR code.
  2. Name it after where it goes up, for example "Yard sign — Elm St".
  3. Choose what It is on a: Yard sign, Flyer, Door hanger, Truck wrap, Business card or Other.
  4. Choose what It opens: A form (then pick Which form), your Website, your Booking page or a Web address.
  5. Press Make it.

The code opens straight away. Press SVG or PNG to download it for the printer, or Link to copy its address. Under the code you see its scans, how many different people scanned it and how many became leads. Leads who reach a form from the code get their own source, so you can tell the sign on Elm St from the flyer.

See where your leads come from

The Lead sources tab shows new leads by where they came from over 30 days, 90 days, a Year or your own Dates. The table lists each Source with its Leads, how many were Won and the Conversion, plus Revenue if your position sees money figures. The line under the table says exactly how each figure is counted.

Reward customers who send customers

The Referrals tab is for your customers. (Business partners who are paid on what they sell are affiliates: see how to track affiliates.)

  1. Under The reward, write What they get, for example "$50 off your next visit", and what it is Worth.
  2. Choose which form Their link opens.
  3. Press Save reward.
  4. Open a customer's card and press Make their referral link, then Copy it and send it to them.

Everyone who comes in through a customer's link appears under Referrals, for example "Jordan Lee sent Renee Castillo". It says Lead until the new customer pays, then Owed. When you have given the reward, press Mark given (or Undo if you pressed it by mistake). Draft thank-you writes a thank-you email to the customer who sent them and opens it for you to check and send.

If it doesn't work

  • "A form needs a name field and an email or phone field, so you can reply."
  • "Give the form a title."
  • "Make a form first." — a QR code or a referral link can only open a form once you have one.
  • "Publish your website first." or "Switch online booking on first." — shown on the Website and Booking page choices until they are set up.
  • "Enter a full web address starting with https://."
  • "A reward is given once the person they sent has paid."
  • "Could not copy. Select it and copy by hand."
  • Deleting a form stops its link working; the leads it brought in stay. Deleting a QR code stops printed copies working.
